[The present chapter covers a wide spectrum of topics related with the diagnosis and treatment of temporomandibular disorders (TMD). As in the case of the previous section on the epidemiology and etiology, some example investigations were chosen in the attempt to keep the readers updated with the current clinical, and not only statistical, concepts on TMD practice. In the field of diagnosis, interesting methodological strategies to discuss were found in the literature on temporomandibular joint (TMJ) imaging, which is suitable to provide examples on how to test diagnostic accuracy of less-frequently adopted imaging techniques such as ultrasonography (US), as well as in the studies on instrumental devices, which example the way some common beliefs did not pass the filter of evidence. In the field of treatment, some studies by the authors’ research group were commented to illustrate some of the different kind of investigations that can be performed and the possible approaches that can be adopted to design a research protocol. In these specific cases, the authors’ experience with TMJ arthrocentesis and injections protocols is used as a starting point to comment on the different study design and, despite not being fully representative of the full spectrum of treatment modalities in the field of TMD and orofacial pain, it provides an evidence-based framework for reading and designing investigations on TMD treatment.]
